To explore how nurses manage personal and professional boundaries in caring for seriously ill children and their families.
Design and methods
Using a constructivist grounded theory approach, a convenience sample of 18 registered nurses from four practice sites was interviewed using a semi-structured interview guide.
Results
Nurses across the sites engaged in a process of maintaining integrity whereby they integrated two competing, yet essential, aspects of their nursing role – behaving professionally and connecting personally. When skillful in both aspects, nurses were satisfied that they provided high-quality, family-centered care to children and families within a clearly defined therapeutic relationship. At times, tension existed between these two aspects and nurses attempted to mitigate the tension. Unsuccessful mitigation attempts led to compromised integrity characterized by specific behavioral and emotional indicators. Successfully mitigating the tension with strategies that prioritized their own needs and healing, nurses eventually restored integrity. Maintaining integrity involved a continuous effort to preserve completeness of both oneself and one's nursing practice.
Conclusions

This paper describes the homebound patients with high symptom burden and healthcare utilization who were referred to an NP co-management intervention and outlines key features of the intervention. We compared demographic, clinical, and healthcare utilization data of patients referred for NP co-management within a large home-based primary care (HBPC) program (n = 87) to patients in the HBPC program not referred for co-management (n = 1027). A physician survey found recurrent hospitalizations to be the top reason for co-management referral and a focus group with nurses and social workers noted that co-management patients are typically those with active medical issues more so than psychosocial needs. Co-management patients are younger than non-co-management patients (72.31 vs. 80.30 years old, P < 0.001), with a higher mean Charlson comorbidity score (3.53 vs. 2.47, P = 0.0001). They have higher baseline annual hospitalization rates (2.27 vs. 0.61, P = 0.0005) and total annual home visit rates (13.1 vs. 6.60, P = 0.0001). To evaluate the effectiveness of standard-of-care oral care guides developed specifically for children in intensive care to prevent mucositis.
Design and Methods
This prospective, interventional and single-group study design was performed in the pediatric intensive care unit of a university hospital in Istanbul between January and December 2014. Daily oral care was implemented to pediatric patients in the study group in line with an oral care guide developed by the researchers. Data were collected using the data collection form and oral mucositis assessment scale published by the World Health Organization (WHO).
Results
Oral mucositis occurred in 16 (5.2%) patients in the pre-intervention group and 7 (2.5%) in the post-intervention group, 10 patients had grade 1, and 6 patients had grade 2 oral mucositis in the pre-intervention group, and in the post-intervention group, three patients had grade 1, and four patients had grade 2 oral mucositis. Although more patients in the pre-intervention group had mucositis than in the post-intervention group, the differences were not statistically significant (P = 0.067).
Conclusions
Oral mucositis can be reduced through the practice of administering oral care in accordance with oral healthcare guidelines.
